Why Prescribed
To treat severe cases of orthostatic hypotension (extremely low blood pressure, causing symptoms of dizziness and faintness, especially when rising from a seated or lying position). Used when standard care, such as support stockings, fluid expansion, and lifestyle changes, is ineffective.
How It Works
Midodrine causes constriction of the smooth muscles surrounding the blood vessels. This effect narrows the width of the blood vessels, causing blood pressure to rise, thus helping to correct low blood pressure on standing.
Range and Frequency
To start, 10 mg, 3 times a day during waking hours, at intervals of 4 hours. Suggested dosage schedule is to take the medication shortly after rising, then at midday, and then late afternoon (before 6 pm). Blood pressure should be monitored while sitting and lying down at the outset of treatment for possible adjustment of doses.
Onset of Effect
Within 1 hour.
Duration of Action
From 2 to 3 hours.

Storage
Store in a tightly sealed container away from heat, moisture, and direct light.
Missed Dose
Take it as soon as you remember. If it is near the time for the next dose, skip the missed dose and resume your regular dosage schedule. Do not double the next dose.

The decision to stop taking the drug should be made in consultation with your physician.
Prolonged Use
See your doctor regularly for measurements of blood pressure while sitting and lying down if you must take this drug for a prolonged period. Periodic tests for liver and kidney function will be done.

Driving and Hazardous Work
Do not drive or engage in hazardous work until you determine how the medicine affects you.
Alcohol

Pregnancy

Infants and Children

Special Concerns
The last dose of midodrine should be taken at least 3 hours before bedtime. Use of this drug is not recommended for patients who have persistent and excessively high systolic blood pressure.
Overdose Symptoms
Goose bumps, sensation of coldness, decreased urine output, rapid heartbeat, ringing in the ears.
What to Do
It is not known whether an overdose of midodrine is life-threatening. However, if someone takes a much larger dose than prescribed, call your doctor, emergency medical services (EMS), or the nearest poison control center immediately.
Drug Interactions
Consult your doctor for specific advice if you are taking cardiac glycosides, phenylephrine, pseudoephedrine, beta-blockers, phenylpropanolamine, dihydroergotamine, fludrocortisone acetate, prazosin, terazosin, doxazosin, metformin, cimetidine, ranitidine, procainamide, triamterene, flecainide, quinidine, or any other over-the counter or prescription drug.

Disease Interactions
Caution is advised when taking midodrine. Consult your doctor if you have any of the following: urinary retention problems, severe heart disease, diabetes mellitus, vision problems, glaucoma, pheochromocytoma, or thyrotoxicosis. Use of midodrine may cause complications in patients with liver or kidney disease, since these organs work together to remove the medication from the body.

Itching, numbness, or tingling, in the extremities, goose bumps, chills, urinary difficulties.

Headache, sinus pressure, redness of face, confusion, dry mouth, nervousness, anxiety, skin rash, vision problems, dizziness, dry skin, backache, gastrointestinal distress, gas, leg cramps.
